(This takes 30-45 minutes), completing this questionnaire allows us to get to the core of your health concerns quickly. During your consultation we will go thru the questionnaire results and make further enquiries where necessary. We will discuss appropriate dietary and lifestyle changes as well as considering testing and recommending supplements where appropriate. |
